Getting Ready for Your First Appointment

A person may seek service from Nimel Mental Health LLC as a self-referral or as a referral from a physician, the courts, family members, or other agencies. People seeking help will be admitted to treatment based upon their individualized need(s), which is determined by screening or assessment. Listed below are the first few steps on your path to wellness.

Prior to the Appointment

Appointments are scheduled with a licensed clinician and are approximately forty-five minutes to an hour-long. Before your appointment, you will be sent a link via email, so you can set up your account in our Patient Portal. There you will complete the following forms: Demographic information form, Patient Confidentiality, and HIPPA forms, Pre-Assessment form, Depression and Suicide Assessment. Because health services are confidential, your authorization for us to share medical records with, and to request medical records from your past and current providers will be requested. 

Intake Assessment

Intake is an interview that usually happens during the first appointment. It is a full assessment of symptoms, medical treatment history, living situation, social supports, and financial resources. This information will be gathered and entered into your electronic health record and used to help determine the right course of treatment. If you would like, a family member may attend part, or all, of the intake meeting. This decision rests with the person receiving treatment. Family members can often be the ONLY source of critical details needed to make an effective treatment plan.

Treatment Plan

The clinician and client work together to develop an individual plan of care; allowing for customization. Information gathered during the assessment is utilized to help develop the treatment plan. The plan outlines the treatment objectives, the recommended therapeutic services, and other activities. The plan also contains the client’s responsibilities, identification of the person responsible for service, and how progress will be measured.
